Admissions

Use this form pre-enrol by date as advised by Campus Central.  Use this form post-enrol by date for administrative errors.

Exams and assessment

Application for Secondary Assessment (Online Form)
For students who believe that unexpected or exceptional circumstances significantly affected their performance in the final examination/assessment item or their ability to attend the exam or submit the final assessment by the due date.  This application form requires supporting documentation.  If necessary, download the UniSA Medical Certificate (PDF) to upload with your application.

For completion by a registered medical/health practitioner when students require supporting evidence of medical circumstances.

Graduations and transcripts

You can order an Academic Transcript,  Australian Higher Education Graduation Statement, Replacement Parchment or postage of your Award Parchment.

Please note that requests for pre-1991 records may take up to 3 weeks to prepare.

Include a contact name and faculty/department for transcripts being sent to other tertiary institutions.

Please consider delivery times for standard international airmail when choosing your postal options.

Do not order an academic transcript / Australian Higher Education Graduation Statement (AHEGS) if one of the following applies to you:

  • You are awaiting grade release. Your academic transcript will reflect your academic record at the time it is issued. If you apply for a transcript before your grades are released the grades will not appear on the transcript.
  • You graduated from January 2018 as your academic transcript, AHEGS and parchment are available via My eQuals unless:

    • you require a printed academic transcript

    • to update your academic transcript to include additional study

  • You are about to complete your program. Upon completion a digital academic transcript will be issued for you at no cost.
  • You are about to graduate. A digital parchment, academic transcript & AHEGS will be issued for you at no cost.
  • Do not apply for AHEGS if you have not graduated from a program at UniSA. AHEGS are only available to those who have graduated from a UniSA program since December 2010.

Do not apply for Postage of Parchment if one of the following applies to you:

  • You have not graduated from a program at UniSA.
  • You are about to graduate and have selected via myGraduation to have your parchment posted to you. An invoice for the postage fee will be issued to you via myFinances.
  • You have received your parchment at your graduation ceremony. If you have lost your original parchment you can apply for a replacement by completing the application available when you select the order here button.

Hire of Academic Dress for your graduation ceremony 
  • Go to myGraduation to hire academic dress for your graduation ceremony and select the appropriate option in myPreferences. An invoice will be sent to your myFinances.

Leave/withdrawal from program

Leave of Absence or Withdrawal from Program (Online Form)
For Australian citizens, Permanent Residents or Temporary Residents (not on a student visa) and for UniSA Online International Students.

Overseas professional registration

This is  for graduates who are applying for professional registration overseas. Preparation of documentation can take up to 6 weeks to complete.
