NOTE: Requests will only be accepted from a UniSA staff member.

I understand and acknowledge that if the Data Point is currently active there is no cost associated with a change in VLAN.

I understand and acknowledge that if the Data Point is currently not activated the above specified Cost Centre will be charged $185. This cost covers the cost of the Networking equipment servicing the data outlet.
