COVID-19 Forms and Checklists

To manage the current working arrangements and potential exceptional leave requirements, several resources have been created including the following COVID-19 related forms. Please also refer to the relevant information via the COVID-19 Information for Staff page.

If staff members need to access personal leave due to COVID-19, the University has introduced a new leave 'reason' within our myUniSA online staff portal. Guidance is provided on the portal itself.
