Application for Correction of Enrolment (staff use only) (Online Form) 
Form used to request amendment to enrolment post-census date due to University administrative error or correction.

Form used when a request has been made to identify and assist a student in confirming their approved credit and remaining program requirements. Less than 12 months of study remain.

Application for Secondary Assessment - SEU Declaration (PDF 24.6KB - download Adobe Reader
This form should be used by SEU Counsellors who wish to provide students with a document to show that they support the students application for secondary assessment.  The student can then upload a copy of this form as part of their application for secondary assessment.  Note that this form is optional, when completing an application for secondary assessment the student can alternatively request that Campus Central contact the SEU counsellor.

Short Program Completion and Certificate Request (PDF 166KB - download Adobe Reader)
This form should be used by University staff only, to request the Completion of Program and Printing of Certificates for Short Program students.
