Staff member missed during Unit WebEx migration

IMPORTANT NOTICE:

UniSA is currently transitioning to a new telephone system. As such, all telephone requests are being assessed by the project team. We apologise in advance for any delays or inconvenience. Please visit the New Telephone System webpage for more information on the new system.


NOTE: This form is only to be completed by staff who are still using a physical NEC phone. If you are a new staff member to UniSA or have never had an extension, you will need to complete the Telephone Work Request form.

NOTE: A new extension number can be provisioned within 3-5 business days. Retaining your existing extension will require you to wait a minimum of 1 month for an Optus migration to occur.

Please ensure your manager is aware that your extension has not been migrated to WebEx and you have completed this form.

 

Terms and Conditions

I understand and acknowledge that if I have requested to retain my current extension number there will be a minimum wait of 1 month after submitting this form for the extension number to be migrated to WebEx via the external vendor.

I understand and acknowledge that headsets are not ordered through ISTS and I will speak to my manager/supervisor to arrange one for my use.
